ADHD What’s Slipping Weekly Reset — One-Page A4 Printable
Some weeks, several routines feel harder to keep up with at once. ADHD What’s Slipping Weekly Reset gives you a place to notice what has been difficult, consider possible reasons and choose your own next steps. Its eight illustrated sections move from checkbox reflection to a three-row action plan, kind reminders and space to record wins. It is a personal planning sheet you can revisit, without needing to complete every box or turn every concern into an immediate task.
This is one original A4 PDF worksheet. Its menus, checklists, reminders and writing areas are parts of that one page, not separate worksheets or bonus files. The artwork uses colorful headings, illustrated everyday objects, icons and blank fields. Choose the parts you want to use and keep the page somewhere convenient for your own planning.
HELPS WITH
• Noticing which routines or responsibilities have felt difficult to keep up with.
• Choosing possible reasons and personal impacts from printed reflection options.
• Considering what kind of support or adjustment you want this week.
• Writing one to three small actions with timing and a personal reminder idea.
• Recording wins and how you would like next week to feel.
REAL-LIFE EXAMPLE
You notice that your evening routine and household tasks have been harder to keep up with. Mark those items if they fit, then consider a printed reason such as too much on your plate. You might choose Simplify or Support and Help in the needs section and write one small action with a time and reminder idea. This is an example of personal reflection, not an assessment of why something happened or a promise that using the sheet changes your feelings or routines.
HOW THE TOOL WORKS
Start with the Week Of and My Focus This Week lines, or go directly to a section that feels useful. The first three numbered panels ask what has been slipping, why it might be slipping and how you feel it affects you. The needs panel asks what would help. A separate plan has three action rows with When and How Will I Remember spaces. Lower sections contain five kind reminders, three trophy lines for wins and prompts for how you want next week to feel.
The printed choices are suggestions for reflection rather than a score or diagnosis. You can select more than one relevant reason, add an Other note or leave an item blank. What you write in the plan is your own choice. This is a single sheet rather than eight separate worksheets, a weekly calendar or an automated reminder system.
The PDF does not assign a task, calculate a score, send reminders or assess your energy or attention. You make the choice and decide how much you want to do. Printed time labels describe optional formats for trying a task; they do not measure how long a task will take in your home.

WHAT IS ON THE PAGE
Week of and weekly focus
Two blank lines let you label the week and write a personal focus. No dates are prefilled, so the page is not tied to a specific calendar year. You supply your own week or date.
1. What’s been slipping?
Thirteen checkbox choices include morning routine, evening routine, self-care, exercise or movement, meals, hydration, sleep, work or school tasks, household tasks, finances or bills, social connections, hobbies or fun, and Other. Choose only what feels relevant; the list does not say that every area needs work.
2. Why is it slipping?
Nine choices include overwhelm, too much on your plate, low energy or burnout, forgetfulness, lack of time, distractions, perfectionism, not being a priority right now and Other. You may choose more than one. These labels are personal reflection prompts, not a clinical explanation or validated assessment of burnout or ADHD.
3. How does it impact me?
Nine options include stress, feeling behind, guilt or shame, increased overwhelm, low energy, less personal time, strained relationships, not meeting goals and Other. Mark an option if it fits your own experience. No score is calculated and no particular feeling is assumed.
4. What do I need right now?
Eight choices include more rest or sleep, more time, simplify, better planning, support or help, encouragement, a guilt-free break and Other. This section makes room for your own support preferences; it does not prescribe care or a treatment plan.
5. My reset plan
Three numbered blank rows invite one to three small actions you choose for this week. Each row includes an action line, a When space beside a clock and a How Will I Remember space beside a bell. These are writing fields, not interactive alarms, calendar integrations or prefilled instructions.
6. Kind reminders for me
Five printed checkbox statements say you do not have to do it all, small steps still count, you can reset anytime, you are doing your best and you may start again. These statements are part of the original artwork and do not promise a change in mood or performance.
7. Wins to celebrate
Three trophy illustrations sit beside three blank lines for your own observations about what went well. The small banner reads No win is too small. You decide what to record; the worksheet does not grade your week.
8. Next week, I want to feel
Six labeled writing lines offer Calm, Focused, Energetic, Organized, Proud and Other. Use them as personal intentions if useful. They are not guaranteed outcomes of completing this sheet.
Encouraging artwork and footer
The upper Reset Reminder note and the lower You Matter, Reset Refresh Realign and You’ve Got This messages are part of the printed design. Teal, pink, purple and orange panels, plants, hearts and everyday icons separate the sections visually. All of this belongs to the same one-page download.
GREAT FOR

• People who prefer choosing from printed options to starting with a blank list.
• Personal weekly routines where a flexible written plan is useful.
• Anyone who likes illustrated prompts, checkboxes and short writing spaces.
IMPORTANT NOTES
This is a practical planning resource, not medical advice, diagnosis, treatment or a substitute for professional support. ADHD wording identifies the product and its intended planning context; it does not mean that the worksheet treats ADHD or guarantees a result. Printed encouragement and suggested timings are part of the original design. No promise is made about attention, energy, mood, confidence, relief or how quickly a routine will change.
Use only tasks and supplies that are suitable for your own circumstances, and follow the directions for any products you already use. The file does not provide individualized health advice or assess why a routine feels difficult. You do not need to complete every prompt or work through every section at once.
This purchase does not include an editable Canva template, interactive form fields, an annotation app, extra paper sizes or additional worksheet pages. Fine lettering and illustration details in generated promotional mockups may differ from the original. The downloaded original customer PDF is preserved unchanged.

I would recommend printing on matte photo paper or smooth card stock, this will give you a more natural look. Soft luster paper is also nice and will add more color saturation. These are also great printed on canvas.
Please know that I have tried to represent the true colors to the best of my ability, however, your actual colors may be slightly different due to each monitor/printer setting.
